PURPOSE: TO GATHER EXPERIENCES AND IDEAS ABOUT DISABILITY ACCESSIBILITY AND INCLUSION FROM ADULTS WITH DISABILITIES AND THEIR FAMILY MEMBERS AND CAREGIVERS

July 12 through July 26, 2021  

  • 100 – 1000 adults with disabilities and family members and caregivers of people with disabilities are expected to participate in the survey
  • There are no costs, reimbursements, or compensations for taking part in the survey
  • The survey will take about an hour and a half to complete. You can take a break and come back to finish the survey later if you choose. There are no known personal benefits to participation, other than providing guidance to the city to improve disability accessibility and inclusion

PUBLIC RECORDS NOTICE: Under Florida law, e-mail addresses are public records. The contact information for the city’s custodian of public records is posted in the city’s primary administrative building in which public records are routinely created, sent, received, maintained, and requested and on the city’s website.
